The Centers for Disease Control and Prevention reported 4,588 confirmed measles cases in the United States as of October 23, 2026. This figure more than doubles the total recorded for the entire year of 2025. The agency attributed the rise to the spread of the virus across at least 43 distinct outbreaks nationwide.

The outbreak timeline shows a rapid acceleration in 2026. Pennsylvania has led the nation in case counts for several months, logging 1,338 infections on the same day. South Carolina and Utah also reported significant clusters, with each state recording more than 500 cases. The CDC added 178 new cases to the national total during the week ending Friday.

Health officials acknowledge that official tallies likely undercount the true prevalence of the disease. Measles is highly contagious, and common symptoms such as fever, cough, and rash often do not prompt immediate medical visits. For unvaccinated individuals, the infection carries a 20% risk of hospitalization. Death from complications occurs in one to three out of every 1,000 infected people.

A discrepancy exists between federal and state mortality data. Pennsylvania authorities reported five deaths associated with measles and are investigating a sixth. The CDC has officially recorded only three measles deaths for the nation. The agency has not disclosed the location of the third death, leaving it unclear if the state and federal counts align.

In September 2026, Health Secretary Robert F. Kennedy Jr. reportedly asked senior CDC officials to remove one death from the official tally. He questioned whether the disease contributed to the patient's death, according to reporting by Reuters. This move added to criticism regarding the agency's transparency during the surge in cases.

The Department of Health and Human Services announced changes to the Vaccine Adverse Event Reporting System, or VAERS, this week. The proposal allows doctors to bill for reporting suspected vaccine injuries. VAERS is a database used by researchers to track safety issues, though it is frequently cited by anti-vaccine activists to argue against vaccine safety.

Kennedy has made several changes to vaccine oversight since taking office. He dismissed all 17 members of the expert panel that evaluated vaccine risks and benefits. A court blocked the appointment of his replacement board members in March 2026. The secretary has also directed the CDC to remove website text stating that vaccines do not cause autism.
